Course overview

This masterclass teaches how to manage, report, and learn from medication errors, by seeing them as opportunities to learn (OTLs).

You will learn the correct reporting requirements to the care regulator: CQC in England, or the Scottish or Welsh Care Inspectorates.

We will show you that high reported error rates are a measure of success, which should in turn lead to a reduction in errors that cause harm.

Managers, nurses and seniors who have a role in managing/investigating medication errors.

Course content

  1. Understand why it’s best to focus on harm reduction rather than trying to reduce medication errors
  2. See errors as opportunities to learn (OTLs)
  3. See how some of the world’s safest care providers achieved a reduction in harm from errors
  4. Create a reporting system to ensure the whole organisation learns from incidents
  5. Outline what an ‘outstanding’ incident reporting system looks like to CQC
  6. Explain which medication incidents need reporting to Safeguarding and CQC
  7. Demonstrate root cause investigation so you can learn from your medicine incidents
  8. Explain how to spread the learning from medicine incidents
  9. Use the Incident Decision Tree to understand when staff need support vs intervention
  10. Receive a template error reporting tool (our OTL form) to use to report medication errors and carry out root cause analysis
