Medicines reconciliation is the act of creating an accurate list of a person’s medicines, and maintaining it. Medicines transcription is then writing these medicines up onto a medicines chart, body maps, when required protocols etc. CQC require that you have had training to do this. This practical, assessed workshop, teaches you how to carry out medicines reconciliation and transcription according to NICE guidelines.

Aimed at

Nurses and senior health and carer professionals working in any health or social care setting (e.g. care homes, supported-living, hospices, community settings).

Course content

  1. Which sources of information can be used to reconcile a person’s medicines?
  2. What checks are needed when those sources disagree with each other?
  3. What additional checks do NICE guidelines require for medicines reconciliation?
  4. What medicines paperwork is considered best practice?
  5. Transcribing medicines onto your own organisation’s medicines charts
  6. Setting up the timing of doses
  7. Transcribing complex dosages (e.g. increasing, reducing, alternate day, weekly doses)
  8. Remote prescribing (verbal orders)
