Medicines reconciliation is the act of creating an accurate list of a person’s medicines, and maintaining it. Medicines transcription is then writing these medicines up onto a medicines chart, body maps, when required protocols etc. CQC require that you have had training to do this. This practical, assessed workshop, we teach you how to carry out medicines reconciliation and transcription according to NICE guidelines. We will design this workshop around the actual medicines charts and other paperwork that you use. As part of this workshop, we can provide you with a medicines reconciliation and transcription policy and medicines reconciliation tool/assessment if needed.
