Medicines Reconciliation and Transcription

Course Overview

Medicines reconciliation is the act of creating an accurate list of a person’s medicines, and maintaining it. Medicines transcription is then writing these medicines up onto a medicines chart, body maps, when required protocols etc. CQC require that you have had training to do this. This practical, assessed workshop, teaches how to carry out medicines reconciliation and transcription according to NICE guidelines.
